Providence Christian Academy works hard to create a safe learning environment as well as an allergy friendly school. We take a host of precautions in order to safeguard the health of each child with a life threatening allergy or a medical condition. Please take a moment to contact me if you have any medical information that would be helpful in caring for your child during the school day this year.
Attached you will find the Allergic Reaction Action Plan, to be completed if your child requires an EpiPen. Also the Diabetic Treatment Plan is attached for those students that require treatment during the school day. I have included the Medication Request Form, to be completed for other medication that your child requires during the school day.
